Great Regulars: That is how I feel about this poem:

"Here" is where "the blockage" is; it is also the present moment (and place) in any of our lives. The "ache" appears to be the ache of ageing--even the poet's doctor says "I have that, we all have". "Here" is also wherever the poet is.
